Your doctor’s plan of care for you can sometimes be hard to follow through on. Often the care-plan or instructions are hard to understand. Your health plan has Nurse Case Managers for those times that your family or a friend are not with you to sort out the course of action your doctor is prescribing. The Nurse Case Managers can work with both you and your doctor. They can help by sorting out your health issues, and get you information to help you better understand your health condition. Your Medicare Advantage plan also provides a 24/7 Nurse line and an in-home visit once a year to help assess any changes to prescriptions or new conditions that may have developed. These visits are with a Nurse or Nurse Practitioner that communicates your current care and may coordinate services you need with the doctor. Often home health needs are assessed during this visit. Follow your doctor’s advice. It is important to your health. You may call the 800 number on the back of your insurance card if you need help using this benefit.
